A brain–computer interface (BCI) is often viewed as a possible path to communicate with totally locked in patients. However, Dr. Birbaumer points out that prolonged complete paralysis might cause the extinction of volitional thinking due to the patient not being able to affect his or her environment. This might cause a problem if patients are asked to learn to interact with a new technique like BCI.
